T HE COMMON denominator in many of the factors influencing long-term renal transplant
survival is the impact of early injury. Although some of these issues are unique to kidney, the
analysis of this problem probably reveals a principle that is applicable to any organ
transplant. In particular, abundant clinical evidence documents that nonspecific injury and
acute rejection are linked to chronic rejection and to late graft loss. The combination of
